The Pout Edit: Redefining The “Natural” Lip With Dr. Michael Brandt

Health & Beauty

In the world of aesthetics, 2026 is officially the year of the “un-done” look. As lip products dominate our beauty bags, the desire for a smooth, hydrated, and perfectly balanced pout has moved from a trend to a total lifestyle shift. We’re moving away from the over-projected aesthetic and toward a “skin-first” philosophy that prioritizes health, structure, and movement.

To get the expert lowdown on how to achieve a look that’s as good in real life as it is on a 4K screen, we sat down with Dr. Michael Brandt of FORM Face + Body, a leading Canadian expert in facial plastic surgery and aesthetic medicine. —Noa Nichol

The “Hydration-First” Philosophy

The days of rigid, “sausage-like” lips are over. Today’s innovation is all about mimicking the body’s own biology to create softness that moves with you.

“We’ve absolutely seen a move away from rigid, over-projected lips toward something softer, more biologic, and more believable,” says Dr. Brandt. “The goal is not to create lips—it’s to restore how healthy lips behave.”

The latest generation of hyaluronic acid (HA) fillers are engineered with advanced cross-linking technologies to behave like native tissue. Because our lips are constantly in motion—smiling, pursing, and talking—the product needs to integrate seamlessly. “Modern HA gels attract water and integrate within the superficial lip tissue, enhancing hydration and texture rather than simply projecting volume,” Dr. Brandt explains.

The “Filter-Free” Reality

While a still photo might look perfect with a “shelf-like” projection, real beauty is dynamic.

“Still photos can be misleading—real beauty lives in motion,” Dr. Brandt notes. “Natural movement means the lips should fold, stretch, and animate without stiffness.”

Achieving this requires a focus on “facial harmony,” which means assessing the lips in proportion to the nose, chin, and dental show. According to Dr. Brandt, “If lips look good mid-laugh and mid-conversation, we’ve achieved the right balance.”

The Structure Secret: Cupid’s Bow & Philtrum

Most people think they need more volume when they actually need architectural refinement. Dr. Brandt reveals that the most overlooked feature is actually the Cupid’s Bow.

“A well-defined Cupid’s Bow creates elegance and lift without adding bulk,” he says. “The philtral columns—the vertical ridges between the nose and upper lip—are equally important. Structure first. Volume second.”

Debunking the “Duck”

The dreaded “duck lip” isn’t a byproduct of filler itself, but rather of poor technique and ignoring anatomy. Modern approaches use strategic “micro-aliquots” placed along structural landmarks rather than flooding the lip body.

“When lips are treated conservatively and proportionately, they don’t look done—they look healthy,” explains Dr. Brandt.

3 Red Flags to Watch For

With the market for aesthetic treatments more crowded than ever, Dr. Brandt warns that lip enhancement should feel like “refinement, not reinvention.” Before you book, keep these three expert tips in mind:

  1. Avoid the “Subscription Model”: If a provider gives you a fixed timeline like “every six months,” be cautious. “Lip treatment should not operate on a subscription model,” says Dr. Brandt. “A thoughtful provider tailors treatment frequency to your anatomy.”
  2. Look for Customization: If every patient receives the same syringe and the same technique, it isn’t medicine—it’s a template. “Not every patient needs projection,” he notes. “Some need refinement, border definition, or improved tissue quality.”
  3. Safety is Non-Negotiable: Most importantly, ask your provider about their emergency protocols. A qualified provider should always have hyaluronidase (the dissolving enzyme) immediately available on-site to manage any potential complications.
